2024-25 forms are no longer available.  If you need assistance with a 2024-25 form, please call 734-462-4433 or come into the Financial Aid Office for assistance.  

The deadline for submitting 2024-25 forms and supporting documentation is 8am on Friday, September 12, 2025.

Loan and Aid Requests

Direct student loans are not automatically awarded to your financial aid award letter. Student loan requests and information on how to apply can be found under “How to Apply” on the loan page.

To request a loan for 2025-26, log in to your Ocelot Access account and, on the Financial Aid card, choose “Request a New Loan”.

To request aid for Spring/Summer 2025, please complete and sign (handwritten signature required) the 2024-25 Spring-Summer Aid Request form and either email it to [email protected] or drop it off in the Financial Aid Office. Watch your SC email for your updated offer letter.

IRS Tax Transcript Information

This form is for students and/or parents to submit directly to the IRS. Any 4506-T forms sent to Schoolcraft College will be shredded.
