These forms are intended to be used to report or request information from an international advisor. You can submit the forms via email to [email protected] or directly to the International Center (McDowell 119). Paper copies of these form are also available in the International Center. 

• Change of Address Form (PDF):
  Report a change of address
• F-1 Extension of Study Form (PDF):
  Request an extension of study
• Letter Request Form(PDF):
  Request a letter for a family visit, social security number, enrollment verification or other reasons
• Reduced Course Load Request Form (PDF):
  Request a reduced course load for a semester
• Reinstatement Request Packet (PDF):
  Determine your eligibility and request reinstatement

Employment Forms

• OPT Reporting Form (PDF):
  Use this form to report within the first 10 days if the start date indicated on your EAD card even if you are not currently employed. Also use this form to report any changes to your address or employment status within 10 days of the change while on OPT.
