Information<\/h2>\n\n\n\n<p>When you are ready to sign up for the TEAS exam, please complete the form below, and submit it to the Testing Center either in person or by email, fax, or USPS.&nbsp;<\/p>\n\n\n\n<p class=\"cta-button move-up-25\"><a href=\"\/testing-center\/wp-content\/uploads\/sites\/16\/2023\/08\/TEAS-reg.-and-pmt.-form-revised-July-2023.pdf\">TEAS Registration &amp;
