Five Levels of Headings
If you have five levels of headings in your paper, it should be typed using the following format:
CENTER FIRST LEVEL HEADING USING ALL CAPITAL LETTERS
Center Second Level of Heading Using Upper and Lower Case Letters
Center Third Level of Heading Using Upper and Lower Case Letters; Italic Type
Left Justify Fourth Level of Heading Using and Lower Case Letters; Italic Type
Indent fifth level of heading using all lower case letters; italic type; ending with a period.
